Both XT and Saxon have extensions to do this, xt:document and saxon:output respectively. The XT extension is described at http://www.jclark.com/xml/xt.html, but all possibilities are described in Chapter 10 of Mike Kay's book "XSLT Programmer's Reference". Get it, it's good.
