Use <xsl:attribute> instead to set an attribute on the open element.

Try

<INPUT TYPE="CHECKBOX">
<xsl:attribute name="NAME">item1</xsl:attribute>
<xsl:value-of select="@label"/>
</INPUT>

instead, and then replace item1 with your xsl:value-of.
