How about modifying the Meunchian method (NB not tested):

<xsl:sort select="@entry" />
<xsl:if test="@entry">
<p><xsl:value-of select="@entry" /></p>
</xsl:if>
</xsl:for-each>

<xsl:key name="entryval" match="index" use="@entry" />
