xsl:template must be a top-level element, i.e. a child of the
xsl:stylesheet element. Here, you've got it as a child of the
xt:document element. Hence the error message. You'll need to restructure
your stylesheet so that all the xsl:template elements are top-level. It
can be done, just takes some thinking. :)
If you post a snippet of your source XML, as well as the XSLT, someone
on the list could probably provide more specific suggestions.
