Declare the table header ro in a fo:table-header element:
<fo:table>
<fo:table-header>
<fo:table-row>
<fo:table-cell border-style="solid" border-width="1pt">
<fo:block text-align="center">Foo</fo:block>
</fo:table-cell>
<fo:table-cell border-style="solid" border-width="1pt">
<fo:block text-align="center">Bar</fo:block>
</fo:table-cell>
</fo:table-row>
</fo:table-header>
<fo:table-body>
... body rows ...
