that is incorrect syntax (for XML, irrespective of XSLT) as the SYSTEM
is supposed to be a URI not an MSDOS file path, so

<!DOCTYPE Hierarchy SYSTEM "file:///C:/Program%20Files/IMS%20HEALTH/Dataview%206/DVH.dtd">
