Usually, position() gives what you want:
<xsl:template match='xxx'>
    <xsl:apply-templates select='yyy'/>
</xsl:template>
<xsl:template match='yyy'>
    <xsl:if test='position()=1'>
        ....
    </xsl:if>
    .......
</xsl:template.
