Sorry, my fault - you need to set the $path parameter before you go
into the xsl:for-each, otherwise the context is wrong. Try:
  <xsl:variable name="path"
    select="substring-before(substring-after(@href, '#xpointer('),
                             ')')" />
  <xsl:for-each select="document(substring-before(@href, '#'))">
    <xsl:value-of select="xalan:evaluate($path)" />
  </xsl:for-each>
