You can't create entities using XSLT because they aren't part of the data model. I'm not sure what you mean by "translating a string into an element". Perhaps you mean that you want to markup the input text "bread and butter and jam" as "bread <and/> butter <and/> jam"? That kind of thing is done using recursive templates: see under "replace" in the FAQ.
