Difficult to tell just from this snippet...
(Which FOP-version?)
Try:
<fo:basic-link>
<xsl:attribute name="external-destination">
<xsl:value-of select="@href"/>
</xsl:attribute>
<fo:external-graphic src="images/download.gif">
</fo:basic-link>
Provided that the @href is defined correctly, this *should* definitely work
with FOP 0.20.5.
