You're trying to generate an attribute node into the result tree when the containing element node already contains child nodes. E.g.
<foo>
<xsl:copy-of select="bar"/>
<xsl:attribute name="baz">qux</xsl:attribute>
…
will throw that error if current node has a bar child.
