Can someone explain / point to resources that explain how a xsl namespace
  is resolved and accessed

A namespace is just an abstract set of names (in this case the names of
the instructions in the XSLT programming language) You can't resolve it
or access it.

There is an html page at http://www.w3.org/1999/XSL/Transform but that
is just some documentation for humans, and XSLT system never access
anything at that URI.
