It's clear to me that your understanding of database issues is deeper than mine, so I just provided the information I could and didn't comment on the rest of it. I can write SQL to query and insert into a database, but my experience with databases is too thin for me to be able to make helpful comments on the issues you raised. I did read your post with interest and look forward to both your posts and those of others, as how to create a good XML-to-DB bridge interests me. However, since I couldn't offer meaningful comments on it, I thought I'd clutter the list as little as possible.
