Question: 
To use attribute element, don't you need a TAG?

Example:
<xsl:template match="/report/version[@id]">
                 <tag>
                                 <xsl:attribute 
name="id">bar</xsl:attribute>
                 </tag>
</xsl:template>

As written, this template matches an element node, so the attribute tag 
would need to be wrapped in an element, as you indicate.

However, if the template matched an attribute node, no element would be 
needed, thus:

<xsl:template match="/report/version/@id">
  <xsl:attribute name="id">bar</xsl:attribute>
</xsl:template>

So, not always.
