process title, insert your prolog, then process body

so instead of

                       <prolog>
                       .....
                      </prolog>
                      <xsl:apply-templates select="node()"/>


do

                      <xsl:apply-templates select="title"/>
                       <prolog>
                       .....
                      </prolog>
                      <xsl:apply-templates select="body"/>
