Setting output method=html didn't do anything for this (not that it would, I suppose).

That is an inappropriate use of d-o-e= in my opinion.
When I need to do the same I do the following:
<!DOCTYPE xsl:stylesheet [ <!ENTITY nbsp " "> <!--known for HTML output, not in XML--> ]> <xsl:stylesheet xmlns:xsl="http://www.w3.org/1999/XSL/Transform" version="1.0"> <xsl:output method="html"/><!--use hardwired browser entities--> <xsl:template match="/"> <!-- root rule --> ... <td> </td> ...
