When I started using DocBook, I made a few changes just to see their effect; they were soon commented out but I continued to use the same identifier. I have a concrete example of niggle 3 (the page number being lower on the first page than the rest) which uses nothing but standard stuff. Unfortunately, it is rather large since it must be longer than a single page; it appears below followed by a log of the various programs called to get the .ps file which exhibits the low page number effect, so that the parameters used can be seen. Also, part way through the source there is a comment. If this is uncommented the page number appears in the right place. (The layout of the source file may appear strange but this is because the file is automatically generated.)
