The DSSSL standard (ISO/IEC 10179) was produced in the following way: the authors and editors typed SGML tags/attributes/entities, etc. in a flat file editor. The SGML files were then sent to FrameMaker SGML (used as a batch paginator). The output from FrameMaker was camera-ready pages that were sent to the ISO central secretariat for printing and binding. Hope this answers your questions. I would love to know why you are asking.
