1. simple-page-sequence only allows inline flow objects in headers
2. a table is displayed (not inlined)
3. jade does not allow page-sequence which does what you want

ergo, you are stuck.

I have a local hack which defines a new flow object, capable of containing
displayed fo's, which becomes the rtf header, but it is rtf-specific,
nonstandard, and very far from production quality. However it may point the
way for a cleverer solution.
