Ooo ... it also answers a question I didn't ask on how to get the
output of a program included into the SGML source --- in the Linux
kernel, we used a kludgy pre-processor to swap a token in otherwise
DocBook source, but I see now we could use the entity directly in
the SGML file and simply pipe in the output from the auto-refpage
program. Categorically cool.
