Dear Jeff,

Just a quick comments on BITS Draft.
We need the <sortkey> element in <index-entry> because it is often
difficult to sort <terms> in the Japanese-language
environment.  We often mix English terms with Japanese terms even,
for example.

HiNET
cc<c=c
c+f$g4"
cccf$g4"
PATOLIS
PubMed
cc<cc*c3c/

etc. Possible sortkeys for the above terms are:

c/cc-c#c(
c1c<cc*ccccc
c0c#c!cccc
c1c(c
c
c1c6cc)
c1c<c>c
cc

or sortkeys in English may be:

HiNET
paasonaru kensaku
Bacchi kensaku
PATOLIS
Pubmed
Paamarinku

With <sortkey>, typesetters can create index pages flexibly.
"xml:lang" should be allowed in <sortkey>.
