I could think of a way if you wanted to generate two seperate files,
(method="xml"), something like the following:

<output file="test.xml">
<p>test</p>
</ouput>

<xsl:template match="output">
<xsl:param name="input" select="document(@file)/*"/>
<xsl:document href="@file">
<xsl:copy-of select="$input"/>
<xsl:apply-templates/>
</xsl:document>
</xsl:template>

<xsl:template match="p">
<para><xsl:value-of select="."/></para>
</xsl:template>
