No, you get all the text node children of the current (book) node.
However if you do value-of on (any) node set then you get the string
value of (only) the first node in teh set in document order, so that's
why you only get the first chunk of text. You could use copy-of instead
of value of but that isn't what you want as that will only give you the
text nodes, any text inside an element will be dropped.

Since you only showed your attempt rather than say what you actually
want to get I'm guessing a bit but don't you just want

<xsl:template match="book">
      <fo:block font-size="12pt" font-family="serif" line-height="16pt">   
         <xsl:apply-templates/>  
     </fo:block>
</xsl:template>

the default template for title will just let the text fall through.

</xsl:template>
