Not sure if it is related to the opera failure, but your stylesheet is
actually asking for inconsistent output.

You are asking for
<xsl:output method="html"
but actually very little of the specific linearisation for html can take
place as that applies to elements in no-namepsace and you are generating
elements in the xhtml namespace which should be linearised with the
(default) xml output method.
