When you say add xmlns="http://www.w3.or/1999/xhtml" to the style sheet where, exactly, should these name space assignments be added?
any element that is or is an ancestor of the literal result element or
xsl:eleemnt that is creating the element, so normally you just need this
once (per stylesheet file) on the xsl:stylesheet element. Note if your
stylesheet includes or imports several stylesheets then they all need
the namespace declarations.
