<document xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" xsi:noNamespaceSchemaLocation="../../xml_utils/hcdocs.xsd">
and any other ("included") output documents contain
<document xsi:noNamespaceSchemaLocation="../../xml_utils/hcdocs.xsd">
