But, we cant seem to find a way that will make a simulation created by Virtools send output to another piece of software (probably in C or C++) constantly (in real time)

One solution would be to write (in C or C++) your own plugin to Virtools (called a BuildingBlock or BB in Virtool's world). This BB would then do the controlling of the external device, using the libraries that you probably have. Check out the SDK documentation for how to write a BB plugin.
Another solution would be to send data using TCP/IP to another application that manages the controlling of the device. More complex to write, and depends on the availability of a TCP/IP BB in Virtools (does this exist ?).
