| 2. | Signed Credit Card Agreement. (PDF) Payment information. We will need a signed copy of this, either faxed or mailed. |
| 3. | New Client Form. The authorized contacts, billing and mailing information for your list service. |
| 4. | Completed Web Order Form. This form will provide us with the information we need to set up your list, e.g., your choice of list name and list owner. |
