  It would be the same image on every page.  If possible I would
like the first page to be slightly different so that I can have a
title spanning the entire width of the page before seperating into a
tall vertical image on the left and then more text on the right.

  I would be very interested in playing with your backend since I'm
convinced this can not be accomplished with the RTF backend.  Please
post a URL where I can get your backend and a simple example
stylesheet if possible.
