Ah but if in my book, I missed a footnote on some word, knowing simply it is the
fifth occurrence is not extremely useful without the complete context - the
first chapter, the second paragraph, the fifth occurrence of "xxx".  Right?  If
it is meaningful to me, I need it meaningful in my XSLT processing.
