  After your email, it seems you have a stylesheet yet that makes the
copy.  I guess you are using the Modified Identity Transform pattern. 
So just add a customized template rule for xs:all:
    <xsl:template match="xs:all">
       <xsl:copy>
          <xsl:apply-templates select="@*|text()[1]"/>
          <xs:element .../>
          <xsl:apply-templates select="node()"/>
       </xsl:copy>
    </xsl:template>
