Institutional repository release form?

Dear Listmembers,

 We are still in the process of licensing to set up an institutional
repository, but will have a crop of theses in May that we would like to
include in the repository.  To that end, I was wondering if anyone has
an example of a standard release form for authors?  I've tried looking a
bit online, but haven't come across one. Also, would such a form have to
be evaluated by our university lawyer?
