stick the rows that you want repeated on each page inside an fo:table-header:

<fo:table>
<fo:table-header>
 <fo:table-row>
  .... repeated header row
  </fo:table-row>
</fo:table-header>
<fo:table-body>
  <fo:table-row>
  .... 
  </fo:table-row>
  ....
</fo:table-body>
</fo:table>
