[stella] SQL megalist

Arg, it was too tedious to type in, so I found a way to parse through Giant 
List and then did some clean up.  Unfortunately I wound up losing the 
associations between programmer and game in the process, so that will have 
to be redone.

Here is the beginning of what I have:

This is compiled from the GiantList and the cart list on 2600 Nexus.

2064 records.

I also have a separate programmers table with 1158 records compiled from 
GiantList.
