This is the guide we created for our faculty: http://libguides.montgomerycollege.edu/copyright. This was created in conjunction with a sub-committee of mostly faculty and in response to a "concern" from Middle States. I saw the guide Kevin Smith just sent the list and now I want to review that since he thinks highly of the way it is laid out. I'm also working on a general guide for copyright. If anyone could recommend examples of good pages to use as a model, I'd love to see them. I'm particularly interested in finding video that I could embed, preferably not from the copyright clearance center.
