The use of a role attribute on emphasis is a personal extension,
that's not intended to be a general feature in DocBook. (Well,
role attributes are a general feature but the semantics are up
to the application.)

| If anyone can shed light on what is happening and how I can get title (and,
| hopefully, any others) working as well as emphasis I'd be indebted to them.
| If more information is needed, it will be provided.

You want the ability to arbitrarily decided that some titles should be
formatted differently, or you want to make all of a specific kind of title
(say Sect3 Titles) different?
